At Nightingale Nutrition and Wellness LLC, we respect your privacy and are committed to protecting your Protected Health Information (PHI). This notice explains how we may use and disclose your health information, and your rights regarding that information.

  • Treatment:  To provide, coordinate, or manage your nutrition care, including sharing information with other healthcare providers as necessary.

  • Healthcare Operations:  For quality improvement, training, professional certifications, audits, and legal compliance.

  • Other Uses and Disclosures:  We may use or disclose your PHI without your written authorization only as required or permitted by law, including public health reporting, law enforcement, or to prevent serious threats to health or safety.

  • Uses Requiring Your Written Authorization:  All other uses and disclosures of your PHI, including marketing purposes, require your written authorization unless otherwise permitted by law. You may revoke this authorization at any time.

Your Rights Regarding Your Health Information

You have the right to:

  • Inspect and copy your PHI.

  • Request corrections to your PHI if it is inaccurate or incomplete.

  • Request restrictions on certain uses or disclosures of your PHI.

  • Receive confidential communications in a specific way or at a specific location.

  • Obtain a list of disclosures we have made of your PHI.

  • Receive a paper copy of this Privacy Notice upon request.

Our Responsibilities

  • We are required by law to maintain the privacy of your PHI.

  • We must provide you with this notice of our legal duties and privacy practices.

  • We will abide by the terms of this notice, except as permitted by law.

  • We will notify you if a breach occurs that compromises the privacy or security of your protected health information.
